There are two versions of each form, a Word document which can be filled in electronically and a PDF version for printing and submitting hand written nominations.

The closing date for nominations is 30 July 2015.

Awards will be presented by Clare Balding at the 2015 Gala Awards dinner at Lords Cricket Ground on Wednesday 30 September 2015.
